This is not an AI. It is a lookup table drawn from real post-incident report language. Describe what happened in plain terms, and we try to match it against known translations into governance-compliant phrasing. If a phrase comes back without matches, or a match does not fit, that language is simply not in the dataset yet, or incorrect in the dataset, which is a gap rather than a verdict.
